To make it easier to keep your family smiling, we offer appointments before and after work, easy family block appointments, and treatment with a minimal amount of visits. You can also download our new patient forms (link to new patient forms, which we’ll send soon) and fill them out before your visit to help save you time. We respect you and want to do everything we can to make your time at Summit View Dental enjoyable.

New patient visits don’t have to be lengthy, complicated affairs that take up too much of your valuable time. At your first visit to Summit View Dental, you can expect:

We make a point to reserve plenty of time for new patient visits, because we want to get to know you and let you get to know our team, too. Dr. Foster will thoroughly discuss your dental history with you, as well as your goals for your smile and any specific concerns that brought you to our office.
We will also complete your exam and cleaning during your first visit, so you don’t have to block out more time for a follow-up appointment.
New Patient Forms
We invite you to download, print, and complete our new patient forms (link) prior to your first visit to expedite the check-in process. Adobe Acrobat Reader is needed to download these forms.
If you are unable to complete these forms in advance, please arrive 10–15 minutes before your scheduled appointment so you have plenty of time to do so in our office.
